Forms and Considerations


If you are 18 years of age or older you are considered an adult. As an adult, HIPPA requires Spring Grove Counseling to maintain your privacy and confidentiality. Consequently, adults are responsible for making your own appointments. If for any reason, an adult is unable to fulfill this responsibility, Spring Grove Counseling will need a release in writing designating a responsible party to act in your place.


Parent participation is vital in the creation of treatment plans. Each counselor will manage how the participation takes place. All children under the age of 18 must be accompanied by a parent with legal custody. In instances of divorcement appropriate documentation signed by a judge is required prior to a child becoming a client.


All sessions are paid for at the time of making the appointment. Initial sessions for children can take up to 90 minutes prior to establishing formal diagnosis. Adults should consider 60 minutes an adequate amount of time for an initial assessment unless you have been properly diagnosis in the past and are in possession of appropriate documentation.
